da8e453d fix: review unrecorded skills from their introducing commit and make setup the single first step Review compared a skill or planning record with no recorded outcome against HEAD, so source changes made between authoring and the first recorded review were reported as no change. Compare such items with the commit that introduced them instead, resolved in one git log call. Make `maintainer setup` copy the check-skills.yml workflow when none exists, and treat package-only distribution as the default so `maintainer check` passes without a recorded distribution choice. Accept a single evidence string when recording and point the report hint at .intent/review.json, which review ignores. Fold the release notes into the existing unpublished changesets and raise the Git-backed test timeouts to match review.test.ts.
