fix(ai-persistence): replace a tool result matched by toolCallId in place Skip the toolCallId match when the incoming tool result has no toolCallId, so two id-less results are not treated as one. Replace the matched stored row with the incoming copy instead of dropping it, the same as a match by id. Move the merge-rules comment onto mergeStoredMessages and add direct merge tests.
