fix(ai): finish every started tool before a parallel batch rethrows - A failed run or before-hook no longer leaves sibling tools running past the batch. The error is thrown after every started tool ends. - An onBeforeToolCall abort still runs the calls prepared before it. - inputRequired and subagent interrupts come back in call order. - Drop the per-tool `sequential` flag. `toolExecution: 'sequential'` is the one opt-out. - Document that toolCacheMiddleware does not dedupe identical calls in one turn under parallel execution.
