fix(ai): ask onShouldContinue after the resumed tools, before the next model call A resume runs the pending tool calls, then went to the model with no loop check. Now the middleware onShouldContinue decides, as after any tool phase. false ends the run with no more model calls. So a harness handoff in a part that also waited for a client tool runs the target in the resolve turn, and the agent that handed off does not answer again.
