fix(ai-harness): cancel queued follow-ups after a log failure, and publish an agent run's finished event before its lease write - A log failure closes the session, and the queued follow-ups of an agent chain now end cancelled then too, so close() does not hang. - An agent run publishes its finished event as it settles, before the awaited lease release, so its operation listeners still get it. - The operation listener cleanup timer does not keep a process alive.
