refactor(ai-harness)!: nest the access rules in access.policy access is now { identify, policy, anonymous?, envKeys? }, and a policy is { authorize, listFilter?, members? }. membersOnly() returns a policy, so a host passes policy: membersOnly(). A custom policy is policy: { authorize }. No spread of the rules into access. Callers, tests, e2e routes, docs, and the changeset follow.
