fix(ai-acp): make approvals, cancel, and queued prompts work in the agent - Cancel during a permission request ends the turn as cancelled. The next prompt cancels the open interrupts first, because the thread takes no new input while they are open. - Interrupts the editor cannot answer (sign-in, input form, client tool) are cancelled, and the model continues. - A rejected resolve receipt ends the turn with an error. - The prompts of a session run one after another, so a later prompt cannot overtake a permission request or report out of order. - session/prompt echoes a user_message with the returned messageId. - Data that is not base64 fails with invalid params. - Unknown session ids fail with resource not found. - The agent keeps no session objects; host.open returns the live one.
